How to Provide Evidence of your Vaccination History

As part of our compliance and safer recruitment process, Flourish Medical may request evidence of your vaccination history. This helps us ensure we meet NHS and healthcare provider requirements when placing nurses into clinical settings.

There are several ways you may be able to access your vaccination history in the UK. This page explains all available options and shows you the best way to provide your evidence.

NHS Website or App

Some individuals may access their vaccination record via NHS online services, including the NHS app, using NHS login details.

  • This may allow you to view or download your vaccination record
  • Availability depends on your location and record completeness

How to view your vaccination history in the NHS App

  1. Open the NHS App on your smartphone
  2. Log in using your NHS login details
  3. Select Your health
  4. Tap check your vaccine record
  5. Your vaccination history will be displayed on screen

How to share it with us

  • Take a screenshot of the page showing your name and vaccination details
  • Send the screenshot securely to your Flourish Medical consultant via email or WhatsApp

Important: Please ensure your name and vaccination information are clearly visible in the screenshot.

Other Ways to Access Your Vaccination History in the UK

While the NHS Website/App is the most convenient, we understand that not everyone’s vaccination history shows up within the vaccination history section of the NHS Website/App. Below are alternative options that may be available.

GP Medical Records

You can request your vaccination history from your GP surgery.

  • This may be provided as a printout or digital document
  • Processing times vary and can take several days or longer
  • Some GP practices may charge an administrative fee

This option is acceptable but often slower than using the NHS Website/App.

Paper Vaccination Records

You may have:

  • A COVID-19 vaccination card, or
  • Historical paper records (for example, childhood vaccination documents)

Please note:

  • Paper cards are not always accepted as standalone evidence
  • They may not include all required details
  • They can be lost or incomplete

These are not the preferred option for compliance.

Red Book (Child Health Record)

The Red Book records childhood immunisations and is usually held by parents.

  • This is not suitable for adult compliance checks
  • It does not cover occupational vaccination requirements

What if you cannot access your records?

If you are unable to access your vaccination history using any of the above methods, please contact your Flourish Medical consultant as soon as possible. We will advise you on the next steps and support you through the compliance process.

Data Protection and Confidentiality

Your vaccination history is personal information. Flourish Medical only requests this evidence for recruitment compliance purposes and handles all documents in line with data protection requirements.
