Millions of NHS dental patients across England are set to benefit from improved access to urgent and complex dental care following the announcement of significant reforms to the NHS dental contract.
The Department of Health and Social Care has confirmed the most substantial modernisation of NHS dentistry in years, with changes designed to prioritise patients with the greatest clinical need. The reforms, due to come into effect from April 2026, aim to make better use of the NHS dentistry budget by focusing resources on urgent treatment and long-term care for complex conditions such as severe tooth decay and gum disease.
Under the new model, patients requiring complex dental treatment will be able to access a single, comprehensive course of care over a longer period, rather than attending multiple fragmented appointments. This approach is expected to improve continuity of care for patients while supporting dentists to deliver more effective treatment through a standardised and incentivised payment structure. For some patients, this could result in savings of up to £225 in NHS dental charges.
Access to urgent dental care will also be strengthened. Urgent treatment will become a core requirement of NHS dental contracts, making it easier for patients experiencing severe pain, infection or dental trauma to secure an appointment locally. Dentists will receive fairer incentives to deliver urgent care on the NHS, reducing the need for patients to travel outside their area or delay treatment.
The reforms form part of the government’s wider rescue plan for dentistry, which includes the rollout of urgent and emergency dental appointments, water fluoridation schemes proven to reduce tooth decay, and a national supervised toothbrushing programme for young children. The toothbrushing initiative alone is supporting up to 600,000 children aged three to five this year, with more than four million toothbrushes and toothpastes already distributed.
Speaking on the announcement, Minister for Care Stephen Kinnock said the changes mark the first step towards restoring confidence in NHS dentistry after years of decline. He emphasised that the reforms are focused on putting patients first while supporting NHS dentists by making the contract more attractive and sustainable.
Alongside patient-focused changes, the reforms introduce measures to strengthen the dental workforce. Dental nurses will be encouraged to apply fluoride varnish and provide preventative treatments for children, supported by fairer remuneration. NHS dental teams will also benefit from enhanced learning and development opportunities, annual reviews and additional support for retention, including guidance on contractual terms and sick leave.
The proposals have been broadly welcomed across the dental sector. The Association of Dental Groups has highlighted the importance of addressing underpayment for complex care and supporting urgent treatment, while also stressing the need to tackle workforce shortages. The British Society of Paediatric Dentistry has described the reforms as a positive step, particularly the increased focus on prevention and prioritising children most in need.
The changes follow a six-week public consultation held earlier this year, which received over 2,200 responses. More than half of respondents agreed that the proposals would improve the current NHS dental contract and help practices prioritise care for patients with the greatest need. Legislative amendments are required and will be introduced ahead of the April 2026 implementation date, with NHS England working alongside Integrated Care Boards to develop detailed guidance.
